Medicare Conditions of Participation Patient Rights
We consider you a partner in your hospital care. When you are well-informed, participate in treatment decisions, and communicate openly with your doctor and other health professionals, you help make your care as effective as possible.
Llano Memorial Hospital encourages respect for the personal preferences and values of each individual. While you are a patient in the hospital your rights include the following:
- To participate in the development and implementation of your plan of care;
- To make informed decisions regarding your care;
- To formulate advance directives and to have hospital staff and practitioners who provide care in the hospital comply with those directives.
- To have a family member or representation of your own choice and your own physician notified promptly of your admission to the hospital.
- To personal privacy.
- To receive care in a safe setting.
- To be free from all forms of abuse and harassment.
- To the confidentiality of your clinical records.
- To access information contained in your records within a reasonable time frame;
- To be free from restraints, of any form, that are not medically necessary or are used as a means of coercion, discipline, convenience or retaliation by the staff.
Responsibilities As A Patient
You are responsible for providing information about your health, including past illnesses, hospital stays and use of medications.
You are responsible for asking questions when you do not understand information or instructions.
If you believe you cannot follow through with your treatment, you are responsible for telling your doctor.
You and your visitors are responsible for being considerate of the needs of other patients, staff and the hospital.
You are responsible for providing information for insurance and for working with the hospital to arrange payment when needed.
Your health depends not just on your hospital care but, in the long run, on the decisions you make in your daily life. You are responsible for recognizing the effect of life-style on your personal health.
If you or your family has a concern, you are encouraged to:
- Speak with you nurse or supervisor on the department where you received a service.
- Contact the Nursing Supervisor through the hospital operator.
- Contact the Patient Representative, extension 2265.
- Contact the Hospital Administrator, extension 2224.
- Contact the Privacy Hotline for any breach in confidentiality, extension 2401 or 325 247-7885.
- Raising a concern will not compromise your current of future care at Llano Memorial Hospital.
Ethics Committee
Llano Memorial Hospital has Social Services available to you and your family should you and/or your physician have difficulty with treatment decisions or end of life decisions. Please ask your nurse or your physician to help you bring these concerns to Social Services.
